General inquiries only
The consultation form requests a name, email address, optional phone number, and inquiry topic. Do not submit health information, private client details, or confidential documents.
How inquiries are handled
Form submissions are sent to the agency’s configured email address so the office can respond. Inquiries are recorded privately in WordPress for administrator review and scheduled for deletion after 30 days. Email and hosting backups may retain copies longer. Email and hosting providers may retain delivery records.
Technical information
The form uses a short-lived, hashed network-address key to limit repeated submissions. WordPress, the hosting provider, and installed plugins may process additional technical information. Contact the office with questions about the live website.
