Employer-completed employee notices
These official templates require employer-specific information and direct delivery. Downloading a blank copy does not complete an employee notice. Timing, handbook, and language requirements.
ESST employee notice
Employer completes the leave year, accrual/frontloading method, and use procedures.
Employee wage notice
Complete pay, employer, deductions, leave, and other required information; give the employee a copy.
